Required Physical Examinations
Pennsylvania requires a physical examination to be submitted to the school nurse three separate times during your child’s education: on entry (usually Kindergarten or 1st grade or new to the district), 6th grade, and 11th grade. If your child receives an immunization during the visit, please ensure that the immunization is documented on the physical form.
Parents/guardians have 60 days from school entry to provide a private physical examination report or signed permission for a school exam; failure to comply with this policy may result in school exclusion and referral to Children, Youth and Family. Parents/guardians will be notified of missing documentation and possible exclusion if conditions are not met.
WCASD is required to offer your child the option of having the physical examination performed at school. There is no charge for this exam. Please note that no required immunizations are given at physicals performed by district consulting physicians. Other options to meet these exam requirements include nearby private walk-in clinics and other facilities. Your school nurse can help with this specific information. If you need assistance with accessing medical insurance for your student, please contact the school caseworker who can help families navigate the various insurance programs.
WCASD utilizes FamilyID for athletic programs; FamilyID is a secure registration platform that provides an easy, user-friendly way to register for our programs and helps us to be more administratively efficient and environmentally responsible. When a student is registered through FamilyID, the system keeps track of information in your FamilyID profile. Information is entered only once for each family member and can be applied for multiple uses and multiple programs. Sports physicals are uploaded through FamilyID. Please provide a separate copy of the physical exam to your school nurse. Please see your home school's athletic webpage for details.
