  • Important Information

11th Grade Physicals are MANDATORY.

  • According to PA State Health code:  ALL 11th GRADE STUDENTS MUST SUBMIT A PHYSICAL EXAM TO THE NURSE'S OFFICE DURING THE FIRST 60 DAYS OF SCHOOL.

    PHYSICAL FORMS are available on the Health Services tab to the left

    What qualifies as a PHYSICAL EXAM? This can be completed at your pediatrician's office, an Urgent Care, PIAA sports physicals, camp or driver license physicals.


PHYSICAL EDUCATION CONCERNS

  • If your child becomes injured or too ill to participate in PE, please send in a physician's note with their diagnosis and specific limitation. After 2 weeks, a Modified PE Form must be completed.

DAILY PRESCRIPTION & OVER THE COUNTER MEDICATIONS

  • Prescriptions and OTC meds sent in by a parent will require a doctor's written order to do so. All medicines must be in their original container, have medication administration form completed, and a parent permission note to administer is still required.

    The only exceptions are Tylenol, Advil, Benadryl, and antacids.


ALLERGY ALERT

  • Students with LIFE THREATENING FOOD, LATEX or BEESTING ALLERGIES need to bring signed MD orders, an allergy plan, and their EpiPens to the nurse's office at the beginning of the year. 

    Students may CARRY their INHALERS and EpiPens with them once they provide MD and parent approval to do so.

REQUIRED IMMUNIZATIONS

  • 4 DOSES OF TETANUS DIPTHERIA
    3 DOSES OF POLIO
    3 DOSES OF HEPATITIS B
    2 DOSES OF MMR- (Measles, Mumps, and Rubella)
    2 DOSE OF VARICEllA (Chicken Pox)
    1DOSE of MCV4 (MENINGOCOCCAL)
    SECOND DOSE (MCV4) PRIOR TO 12th GRADE
    1DOSE TDAP (Tetanus Diphtheria  Acellular)


MANDATED SCREENINGS

  • ALL GRADES receive height, weight and vision screening during the first few days of the school year.

    ALL 11th GRADERS have a hearing screening by state law.

    RESULTS ARE AVAILABLE  on Power School under HEALTH SCREENINGS. 

    *If your student fails any of our screenings, please have them evaluated by your vision or hearing specialist. Your pediatrician can discuss weight concerns with you at a follow-up appointment.
